IN ORDER TO COMPLETE ONLINE REGISTRATION, HARD COPIES OF CERTAIN DOCUMENTS AND PROOF OF RESIDENCY WILL BE REQUIRED TO BE UPLOADED DURING THE PROCESS.

Please print and complete the following forms for each student PRIOR to beginning online registration. If you have any questions, contact Central Registration at (516) 765-4165.

ALL REQUIRED FORMS WILL NEED TO BE SCANNED AND UPLOADED DURING THE PROCESS. 
PDF or photo formats (TIFF, JPEG, PNG, GIF) will be acceptable.
